  • Performing Annual wellness check-up calls, once the patient completes their annual physical examination with the PCP.
  • Offer CCM (Chronic Care Management) services for diseases such as Hypertension, Diabetes, Hypercholesterolemia, Hypertriglyceridemia, Hypothyroidism, and other NCDs.
  • Exercise data entry in Electronic medical records used by healthcare professionals abroad such as IMS, EPIC, and E-Clinical Works.
  • Continually develop a network of quality providers and keep accurate patient records, coordinating appointments, and managing care plans.
  • Help patients achieve their necessary healthcare goals, both- short-term and long-term.
  • Establish proper & effective communication and prospective relationship with patients.
  • Bring the patient’s questions and concerns directly to the attention of the PCP.
