    Care Manager

    We are looking to welcome a Care Manager to join India's first comprehensive multidisciplinary phygital pain management company.

    The Care Manager will be a key part of the senior leadership team, being the single point of contact for the patients along their chronic pain management journey, while ensuring the company's broader vision and goal.

    Here is a snapshot of the responsibilities:

    • Conducting patient onboarding calls to understand their goals and preferences
    • Coordinate the care of patients with the clinical team, including scheduling appointments, managing medications, and facilitating communication between patients and the clinical team.
    • Educate patients and families about pain management techniques and strategies, facilitate medication management, care sessions, lifestyle modification and self-care techniques
    • Maintain accurate and up-to-date patient records, including documentation of care plans and progress notes.
    • Gather and organize care program feedback from patients and the clinical team
    • Actively participate in improving the program's protocols, systems and processes to have better patient and provider outcomes.

    Skills & Experience sought for this role:

    • 0-1 years of consumer-facing hospitality experience as health coach/Habit coach/Air hostess/Front desk attendant.
