Pharmacy FullTime Day MonFri84pm Position SummaryUnder the general supervision of the Director of Pharmacy Servicesthe Clinical Pharmacist will assume theresponsibility and accountability for themanagement of rational antibiotic therapy used within our Hospitalor management of the critical care or highrisk patients as defined by area of responsibility.

Will providefor the collaboration and consultation withother health professionals on the therapeutic use of antibiotics orother medication for patients under the scopeof responsibility.

Will also provideeducation and training for health professionals involved in thisclinical forum.

Essential Functions andResponsibilities Includes the Following: 1.Understands and adheres to the WPH Performance Standards Policiesand Behaviors. 2.

Improve the monitoring andmanagement of the clinical pharmacy antibiotic service or ofcritical care or high risk groups to includeinterventions to improve the quality and cost effectiveness ofpatient care.

3. Promotes the clinicalpharmacy development of pharmacy staff by providing clinicalleadership and education asappropriate. 4.

Work collegially with othersin all activities in order to achieve organizational objectivescooperating in both interpersonal and teamrelationships to establish common ground build shared goals andencourage new ideas.

5. Generate creativeapproaches and new ideas to improve performance service andquality.

This includes identifying andweighing options and interventions involving appropriate people inthe decision making process and making sounddecisions after reviewing all relevantinformation.

Takes personalresponsibility for all work activities and personal actionsincluding following through on commitmentsimplementing decisions that have been agreed upon and acknowledgingmistakes or failures without blamingothers.
