Description

Care Coordination:
Serve as the primary point of contact for patients, their families, and healthcare providers.
Coordinate and facilitate patient appointments, referrals, and consultations.
Collaborate with various healthcare teams to develop and implement individualized care plans for patients.
Ensure that patients receive appropriate and timely medical interventions, tests, and treatments.
Monitor patient progress and follow up on any necessary interventions or adjustments to the care plan.

Patient Advocacy:
Advocate for patients' rights, needs, and preferences throughout their healthcare journey.
Address patient concerns, complaints, or issues related to their care.
Communicate effectively with patients to ensure their understanding of their medical conditions, treatment options, and care plans.
Provide support and guidance to patients and their families, including education about available resources and support services.

Communication and Collaboration:
Facilitate effective communication between patients, healthcare providers, and support staff.
Coordinate multidisciplinary team meetings and ensure the exchange of relevant patient information.
Collaborate with healthcare professionals to develop and implement strategies for improving patient outcomes and experiences.
Act as a liaison between patients and insurance providers to assist with insurance authorizations, billing inquiries, and coverage issues.

Documentation and Record-Keeping:
Maintain accurate and up-to-date patient records, including medical histories, treatment plans, and progress notes.
Ensure compliance with legal and regulatory requirements for patient data privacy and confidentiality.
Document and report any incidents, adverse events, or patient safety concerns according to organizational policies and procedures.

Quality Improvement:
Participate in quality improvement initiatives to enhance patient care processes and outcomes.
Collect and analyze patient satisfaction data to identify areas for improvement.
Collaborate with healthcare teams to develop and implement quality improvement plans.

Salary:
₹15, ₹35,000.00 per month
