Overview:


As a Patient Care Assistant at RxPONT, you will play a pivotal role in providing comprehensive patient support and assistance.

Your responsibilities will encompass managing the entire patient journey from enrollment to ongoing support.

You will be the primary point of contact for patients, ensuring they receive the necessary care and support throughout their engagement with RxPONT.

This role demands excellent communication skills, a robust background in healthcare, proficiency in multiple languages, and the ability to handle diverse aspects of patient care.


Key Responsibilities:

Patient Enrollment and Information Provision:

Attending Patient Calls:
Receive inbound patient calls on the toll-free patient line. Engage with patients who are seeking to enroll in specific patient support programs.

Obtaining Patient Consent:

Skillfully guide patients through the consent process, ensuring they understand the implications and benefits of participating in the program.


Disease-Specific Information:
Provide patients with accurate and approved disease-specific basic information, addressing their questions and concerns.


Patient Registration and Documentation:

Patient Registration:
Register patients in the appropriate patient support program, ensuring all necessary information is accurately recorded.

Document Collection:
Efficiently collect and organize all relevant patient documents, including prescription copies, diagnostic details, and signed consent forms.

Patient Support Plan:
Collaborate with the patient to create a tailored support plan, aligning it with program requirements and patient needs.


Medication Adherence and Support:

Medication Adherence Calls:
Regularly reach out to patients to monitor and support medication adherence. Address any issues or concerns patients may have.

Customized Diet Plans:
Provide personalized diet plans to patients as required, in consultation with program guidelines.

Field Colleague Coordination:
Facilitate communication and coordination with field colleagues who visit patients for medical device training and treatment initiation.


Collaboration and Coordination:

In-House Program Management:
Act as a vital liaison with in-house program managers to ensure program objectives are met. Provide regular updates on patient status.

Medication Access:

Work closely with the in-house medication dispatch department and external medicine distributors to ensure patients have seamless access to medications.


Client In-Charge Office:

Maintain a strong connection with the client in-charge office, providing essential program health reports and necessary Management Information System (MIS) reports.

Qualifications:

Qualifications:

Healthcare Experience:
Previous experience working in a healthcare organization in a support function is a prerequisite.

Language Proficiency:

Exceptional verbal and written communication skills in English, Hindi, and at least one regional language (e.g., Tamil, Kannada, Telugu, Marathi) are essential.


Technical Skills:
Proficiency in using standard office software, including word processing and data management tools.

Detail-Oriented:
Strong organizational skills and an unwavering attention to detail are necessary for accurate patient record-keeping.
