Nutritionist for Patient Counselling - Delhi, Delhi, India - Portea Bureau
Description
Key areas of responsibility:
- Provide counselling related to the product, its usage, side effects and treatment adherence.
- Provide counselling and psychosocial support to heart failure patients and their families.
- Develop individualized care plans and schedules to help manage their condition.
- Assist patients and their families with understanding their condition and how Cidmus can help them Educate patients and families on lifestyle and medical management of heart failure.
- Coordinate with all stakeholders to ensure all aspects of patient care are addressed.
- Track patient progress, document patient care, and maintain patient records.
- Provide emotional support to family members and other caregivers
- Work with healthcare systems to ensure continuity of care for patients
Qualification:
- Bsc Food & Nutrition / Msc Food & Nutrition
- Language English(mandatory)+ Any Regional Language
Salary:
₹20, ₹27,000.00 per month
