Description

Identification of drug related problems

  • Review patient case records for identification and prevention of medication errors
  • Monitor and report Dispensing, Prescription, Administration errors
  • Monitor and report Drugdrug interactions and drug to food interactions
  • All necessary drug related interventions are reported to respective doctors either through mail or respective patient case record in hospital information system

Drug information services

  • Provide relevant drug information to patients

Patient counselling

  • Provide Patient medication counselling
  • Counsel the patient regarding their disease condition and importance of medication adherence.

Adverse drug reaction monitoring

  • Interact with patients and Monitor patient case records for adverse drug reactions
  • Document all adverse drug reactions occurred during the treatment period within the hospital through "ADR REPORTING FORM"

Antibiotic utilization review

  • Antibiotics usage monitoring with infectious disease specialist
  • Monitor antibiotic utilization and compare the compliance with hospital antibiotic policy

Hospital drug formulary

  • Update and circulate hospital drug formulary to all health care professionals

Salary:
From ₹25,000.00 per month

Experience:
