Clinical Pharmacist
Job Description:
Responsibilities:

Clinical Assessment: Conduct comprehensive patient assessments to determine appropriate medication therapy, taking into account medical history, current medications, and individual patient needs.

Medication Management: Develop and implement medication therapy plans to optimize patient outcomes, including dose adjustments, therapeutic substitutions, and monitoring for adverse effects.

Patient Counseling: Provide patient education on medication use, including dosage instructions, potential side effects, and drug interactions, to ensure safe and effective treatment.

Collaborative Care: Work closely with healthcare providers to coordinate patient care, participate in interdisciplinary rounds, and contribute to treatment decisions.

Drug Information: Serve as a clinical resource for healthcare professionals and patients, providing accurate and timely information on drug therapy, pharmacokinetics, and pharmacology.

Quality Improvement: Participate in quality assurance initiatives to enhance medication safety, including medication reconciliation, adverse drug event monitoring, and performance improvement projects.

Clinical Documentation: Maintain thorough and accurate documentation of clinical interventions, patient assessments, and medication therapy plans in accordance with regulatory requirements and organizational policies.

Continuing Education: Stay abreast of current developments in pharmacotherapy through ongoing education and training activities, including attending conferences, seminars, and professional meetings.
Requirements:
- Bachelor of Pharmacy (PharmD) degree from an accredited institution.
- State licensure as a Pharmacist in good standing.
- Minimum of 2 years of clinical pharmacy experience, preferably in a hospital or healthcare setting.
- Strong clinical knowledge and proficiency in medication therapy management.
- Excellent communication and interpersonal skills.
- Ability to work collaboratively in a multidisciplinary team environment.
- Attention to detail and strong analytical skills.
- Commitment to providing highquality patient care and service excellence.
Pay:
₹10, ₹20,000.00 per month
Education:
- Diploma (preferred)
Experience:
- Basic computer: 2 years (preferred)
- Pharmacist: 2 years (preferred)
