Job Description:


Position Overview:

We are seeking a highly motivated and compassionate Lead Generation and Closing Specialist to join our team and focus on post-discharge patients.

In this role, you will play a pivotal role in identifying potential clients who require home care services after being discharged from hospitals or medical facilities.

Your primary responsibility will be to generate leads, nurture relationships with patients and their families, and guide them through the process of choosing our home care services for a smooth transition to home-based care.


Responsibilities:


Lead Generation:

Proactively reach out to hospitals, rehabilitation centers, and medical facilities to identify potential post-discharge patients who may require home care services.

Collaborate with healthcare professionals, case managers, and social workers to obtain patient referrals.

Relationship Building:
Establish a rapport with patients and their families during their hospital stay or rehabilitation process. Offer empathetic support and demonstrate understanding of their unique needs and concerns related to post-discharge care.

Consultation and Assessment:
Conduct thorough assessments to understand the specific requirements of post-discharge patients and their home care needs. Provide informative consultations to explain our services and how they can benefit the patient's recovery process.

Tailored Care Plans:

Work closely with our caregiving team and medical professionals to develop personalized care plans that address the patient's medical, rehabilitative, and emotional needs.


Coordinate Care:
Facilitate a smooth transition from hospital to home-based care.

Collaborate with the discharge planning team to ensure a seamless handover of care and that all necessary arrangements are in place.


Follow-Up and Monitoring:

Maintain regular communication with patients and their families to monitor their progress and satisfaction with our home care services.

Address any concerns promptly and take proactive measures to enhance their experience.

Sales Process Management:

Utilize and maintain our sales tracking systems and customer relationship management (CRM) tools to manage leads, appointments, and follow-ups.

Keep accurate records of interactions with patients and their families.

Collaboration:

Collaborate with our marketing and outreach teams to develop targeted strategies for lead generation and engagement with post-discharge patients.


Market Research:
Stay updated on industry trends, best practices in post-discharge care, and patient preferences. Use this knowledge to improve our services and tailor our approach to better meet patient needs.

Achieve Sales Targets:
Set and achieve sales targets for post-discharge patient conversion, contributing to the overall growth of the organization.


Salary:
₹18, ₹30,000.00 per month
