Roles and Responsibilities

Explain Diabetes Clinic Package to the patient in brief.
Tracking the status of patients treatment progress.
Collaborates, advocates, and confers other members of the diabetes care team in developing patient centric diabetes plans.
Engages throughout the programm and serves as a role model of professionalism.
Integrates skills and knowledge for selfmanagement of diabetes into clinical practice.
Participates in meetings, serves on committees, and represents the department and hospital/facility in community outreach efforts as appropriate.
Completes comprehensive assessments for each patient including emotional and behavioral health, interprets personal health data, develops an individualized care plan based on the patientassessed needs and goals and promotes successful selfmanagement.
Active participation in improving quality of care and outcomes for those living with, at risk for, and affected by diabetes.
Provide diabetes and self management guidance and health education to Pre
Diabetes ,Type1,Type2 and gestational diabetes patients.
In collaboration with patient and healthcare personals develop a physical activity plan that accommodates patients need, abilities, and self management goals and monitor progress.
Assist patient and their relatives about home care and treatment also give a brief note about the complication that would occur if not plan of treatment followed.
Performs other duties as assigned.
Excellent verbal and written communication skills.
Basic computer skills including use of Microsoft Office.
Has working knowledge of diabetes technology.
Highly organized and strong attention to detail.
Strong problem solving and critical thinking skills
