Duties And Responsibilities:
- Perform And Document Prescription Audit In Both IP And OP Departments.
- Evaluation Of Adverse Drug Reactions Reported And Issuing Of Alert Card To The Patient.
- Patient Counseling On Medications In IP And OP Departments
- Participation In Medication Reconciliation Process To Rule Out Any Drug Interactions.
- Assess and Report Clinically Significant Drug Interactions And Drug food Interactions In Prescriptions.
- Conduct In
- Service Training For Pharmacists On Updated Policies And Procedures For NABH Conduct Educational Programme For Nursing Staffs On Medication Management.
- Updating Of Hospital Formulary
- Analyzing And Recording The Facts Related To An Incident Reports
- To actively participate in THE NABH and all future accreditations.
