Duties And Responsibilities:


  • Perform And Document Prescription Audit In Both IP And OP Departments.
  • Evaluation Of Adverse Drug Reactions Reported And Issuing Of Alert Card To The Patient.
  • Patient Counseling On Medications In IP And OP Departments
  • Participation In Medication Reconciliation Process To Rule Out Any Drug Interactions.
  • Assess and Report Clinically Significant Drug Interactions And Drug food Interactions In Prescriptions.


  • Conduct In

  • Service Training For Pharmacists On Updated Policies And Procedures For NABH Conduct Educational Programme For Nursing Staffs On Medication Management.
  • Updating Of Hospital Formulary
  • Analyzing And Recording The Facts Related To An Incident Reports
  • To actively participate in THE NABH and all future accreditations.

Pay:
From ₹25,000.00 per month
