As part of our commitment to excellence, we are seeking a skilled and compassionate physician to join our geriatric services team.

Responsibilities:

Provide comprehensive medical care to geriatric patients in outpatient and/or inpatient settings.
Conduct thorough assessments and develop individualized care plans tailored to patients' unique needs and preferences.
Manage chronic medical conditions, acute illnesses, and agerelated health issues in collaboration with other healthcare team members.
Coordinate care transitions, including hospital admissions, discharges, and postacute care arrangements.
Collaborate with specialists, therapists, and other healthcare professionals to optimize patient outcomes and quality of life.
Educate patients, families, and caregivers about geriatric health issues, preventive measures, and treatment options.
Participate in interdisciplinary team meetings, case conferences, and quality improvement initiatives to enhance patient care delivery.

Qualifications:

Medical degree from an accredited medical school.
Board certification or eligibility in Internal Medicine, Family Medicine, or Geriatric Medicine.
Valid medical license in the state of practice.
Proven experience in providing primary care to geriatric patients.
Excellent clinical skills, including proficiency in managing complex medical conditions and geriatric syndromes.
Strong communication, interpersonal, and teamwork skills.
Commitment to delivering patientcentered care with compassion and empathy.
Ability to adapt to changing healthcare environments and work collaboratively in a multidisciplinary team.
