Job Description:

Ability to review the patient's longitudinal medical record & triage potential conditions based on symptoms, medications, labs, clinical notes, social & family history among other data points.
Understanding valuebased healthcare in the US particularly key concepts associated with patient risk, major conditions and comorbidities driving healthcare utilization and consumption, and key shifts in value based care.
Conducting deep and thorough evaluation(s) of patient documents to identify missed diagnoses, gaps in care and/or other evidence of risks, diseases and occurrences that could meaningfully increase the patient's risks for a poor outcome.

In short:
Review Patients Medical Records

Review case history of patients & their family

Understanding patients' risks, major conditions, medical coding, etc.

Conduct deep and thorough evaluation of patients documents

Identify missed diagnoses, gaps in care or risk, diseases & occurrences that could meaningfully increase the patients risk.

Identify Symptoms.
