Part Time Monday Friday 9a 5pPosition Summary TheWound Care Therapist practices wound care within the scope ofpractice of physical therapy or nursing which is defined as theevaluation treatment and/or prevention of disability injury diseaseor other conditions of health using physical chemical and/ormechanical means including but not limited to heat cold light airwater sound electricity massage mobilization and therapeuticexercise with and without assistive devices; the performance andinterpretation of tests and measurements to assesspathophysiological pathomechanical and developmental deficits ofhuman systems to determine treatment and assist in diagnosis andprognosis.
Essential Functions andResponsibilities Includes the Following:1.Understands and adheres to our Performance Standards Policies andBehaviors2. Performs wound care for both inand outpatients within the applicable physical therapy or nursingscope of practice whichever is applicable.3.For Physical Therapists (PT):
Perform wound care either under aphysician plan of care or a physical therapy plan of care (asapplicable) which requires an order and includes an evaluation withor without physical therapy evaluations and treatment proceduresupon physician s referral for inpatients on the hospital units andin the Wound Care Center as well as for outpatients in the WoundCare Center.4.
For Registered Nurses (RN):Diagnose and treat human responses to actual or potential healthproblems through such services as case finding health teachinghealth counseling and providing care supportive to or restorativeof life and wellbeing.
RNs also execute medical regimens prescribedby a licensed physician dentist nurse practitioner physicianassistant specialist assistant midwife or podiatrist in accordancewith New York State Education Department regulations.
A nursingregimen must be consistent with any existing medical regimen. RNspractice under the supervision of a physician nurse practitioner orpodiatrist.RNs will:
assess patients identify unmet patient needsand plan comprehensive care to meet those needs in relation to thepatient s illness condition or disability; Identify unidentifiedcomorbidities or emergent complications and intervenes solely andalso with health care team members to initiate additionalexploration and treatment; coordinate care with other health careproviders such as respiratory therapists social workers physiciansnurse practitioners dieticians and discharge planners; and providespatient care (i.e. administer medication and perform nursinginterventions).5.
Works collaboratively withWound Care Center staff including physicians nurse practitionershospital nurses Ostomy/Wound nurses home care nurses andtechnicians to promote interdisciplinarycare.6.
Supports physician and nursepractitioner staff with wound care procedures both in the WoundCare Center and at the bedside in inpatient units includingsurgical debridement incision and drainage aspirations etc.
Removesdressings and cleanses wounds.7. Appliestopical medications to wounds with a physician or nursepractitioner order.8.Performs debridementof devitalized tissue from wounds without anesthesia includingsharp selective debridement with highpressure waterjet scissorsscalpel and forceps with referral from a physician or nursepractitioner.9.
Applies wound dressings andbandaging including compressive wrapping negative pressure woundtherapy etc.10. Adheres to hospitalinfection control policies.11.Carries outregular assessment/reevaluations develops plans of treatmentmodifies treatment plans/programs as necessary; determinesrehabilitation potential and participates in discharge planning forwound care patients.12.
Documentscomprehensive wound assessments including drainage color texturetemperature vascularity condition of surrounding tissue and size ofthe area to be targeted for debridement asneeded.13.
Notes possible need for patientsto receive other services (surgical or infectious diseaseconsultation PT OT etc.). Discusses needs with other professionalsand physicians.14. Knows and utilizes variedtreatment techniques and skills for the problems / disabilitiesencountered.15.Keeps uptodate records ofeach patient s treatment and progress and assigns appropriatebilling codes for services rendered.16.Responsible for scheduling of patients sessions and arranges fortransfer of duties in the event ofabsence.17.
Instructs patients in the useand care of PT related equipment and apparatus such as wheelchairbraces canes crutches walkers prosthetics and orthotics (asapplicable).18.
Directs and supervises theactivities of Wound Care Technicians students andvolunteers.19. Maintains departmentequipment; reports defects or problems immediately to supervisor orappropriate department i.e. BiomedEngineering.20. Consults with other healthrelated departments (i.e. nursing physical therapy occupationaltherapy etc.) to coordinate therapeutic programs and maintainscommunication with physicians and nurse practitioners regarding thepatient s progress and response totreatment.21.Demonstrates and teaches WoundCare therapies to students hospital nursing staff and other staffas determined by the Director.22.Participates in periodic assessment of the quality of careprovided; makes the suggestion for improvement of quality asneeded.23.
Attends department meetingsallied health conferences special courses seminars and applicableinservices. Reports on relevant course material to staff. Sharesareas of special interest or expertise withstaff.24. Accepts rotations in assignmentsto bedside wound care.25. Instructs familiesabout the needs management capabilities and precautions related tothe patient s wound care.26. Providesinformation regarding billing for services andequipment.27.Determines needs for purchaseof equipment for patients and department.28.Maintains and carriers out procedures for departmental recordscharge slips daily statistics etc.

Graduate of an accredited college program in Physical Therapy;B.A./B.S. in Physical Therapy or higher degree or graduate of anaccredited college program in Nursing.
Licensure:
New York State license or eligibility for PhysicalTherapy or Registered Nurse license
Experience:
Successful completion of orientation program accordingto department guidelines
Qualifications:
