    Company Details:

    One of the leading healthcare companies is in the process of setting up a strategic team to bring healthcare closer to patients. This initiative involves establishing primary and secondary care services that are easily accessible to patients, with hyperresponsive processes focused on patient needs and satisfaction. The goal is to bridge the gap between in-hospital healthcare and everyday patient needs, providing a continuum of care tailored to individual healthcare requirements.

    The primary role of the Patient Care Coordinator is to deliver care by coordinating between patients and the provider's daily schedule, as well as through proactive interventions. Acting as a conduit for information exchange between providers and customers, coordinating with medical and support services for all customers, and facilitating information flow and referral services to address medical and psychosocial needs.

    Key Responsibilities:

    • Facilitate care by coordinating with customers for visits and future healthcare needs, ensuring proper care for each customer.

    • Handle customer calls, resolving inquiries promptly or directing to the appropriate department.

    • Support effective communication between customers and medical staff, including relaying messages and gathering information for providers.

    • Assist in medication refill processes to ensure timely and accurate information transfer.

    • Coordinate with clinical and nonclinical support services to facilitate care.

    • Track all customers, enter data into systems for follow-up and reporting.

    • Collaborate with medical staff to provide case management for customers with complex medical or psychosocial needs.

    • Engage in quality improvement and assist in implementing improvement programs.

    • Perform any other duties as assigned by relevant authorities.

    Additional Details:

    Requirements include proficiency in verbal communication in English and Kannada/Hindi, with knowledge of any local language as a plus. Candidates should be Graduate/Postgraduate with 0 to 2 years of experience, showcasing effective teamwork, problem-solving, interpersonal, and customer service skills. Strong organizational, time management, and familiarity with local social service resources are essential, along with the ability to work with diverse cultures and socio-economic backgrounds. Basic knowledge of Microsoft Word and MS Excel, as well as proficiency in computer applications for panel management, are required.
